For New Patients

All patient forms are completed through a Secure Patient Web Portal.  Upon making an appointment, the office will send by email and invitation to the new patient to establish their Patient Portal.  Part of this process is to complete the health and history questionnaires and informed consent forms.  All is done electronically so that the time at the first office visit will be fully devoted to patient care and evaluations.  Patients pay for the doctor’s time and we want all time spend to be on patient focused care.  If the documentation is not completed prior to the first appointment, the appointment may cancelled.
